Use these forms to either:

  • apply to trainer-book theory tests for your pupils

  • apply to change your business details or add, change or remove staff members who can book tests

You need to be registered with the Driver and Vehicle Standards Agency (DVSA) as a business to do this.

Being registered as a business is different to being registered as an instructor.

If you have a business ID from DVSA

  1. Download and fill in the form.

  3. Email the form to ttnationaloperations@dvsa.gov.uk.

If you do not have a business ID from DVSA

Email DVSA to register as a business before you fill in the form.

Use the subject title ‘Register for a business ID to book theory tests for pupils’.

You need to include your:

  • full name
  • address and postcode
  • date of birth
  • telephone number
  • approved driving instructor (ADI) personal reference number or instructor reference number
